Home Care Services (Non-medical)

When to get home care services?
ā
Homecare services can enable them to stay in their own homes or live in a retirement community. Homecare aids can help with non- medical services. We work with our case managers and collect information about each client’s specific needs, develop solutions, and implement an action-oriented individualized care plan. Once this care plan is in action, the care managers regularly meet with clients and their support systems to ensure that each client receives the care needed to maintain their safety.

What to look for to determine if someone needs in home care?

-Activities of Daily living:
If they have difficulty on bathing properly, cooking, managing medications and walking.
-Health Condition:
Changes on appetite, sleeping routine, hearing loss, weight loss, forgets easily.
-Personal Care:
Poor hygiene, poor appearance, bad body odor.
-Daily task responsibilities:
Unpaid bills, forgetting to open regularly mail, clutter and unclean home, no showing for medical appointments, unfilled prescriptions.

Home Health Care (Medical)
ā
We offer referrals for home health care working with accredited home health agencies. Our network of home health agencies accepts all types of health insurance.
This is medical care that's prescribed by your loved one's doctor. It requires a Registered Nurse (RN), Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to administer medication and skilled nursing care. The goal of home health care is to treat an illness or injury, to regain their independence and become as self-sufficient as possible.
Home health care can work in conjunction with other homecare aids who can assist with daily living activities, such as bathing and dressing. Also, home health providers specialized in wound care, injections and caregiver education.

Elder Care Planning-
