The purpose of our elder care planning is to provide a free and no-obligation aging assessment to seniors and their families. We service the community by educating the public and arranging free consultations with our members or with members of our extended networking group.
Our primary objective is to create well rounded solutions for people seeking us out. We do this through an initial assessment to uncover all the aging issues challenging aging seniors. Typically, we work with the children or other supporters of aging seniors, but we can also work with the aging seniors themselves.
We are the only accredited council in San Francisco Bay by the National Care Planning. NCPC

This assessment and the resulting recommendation report are a free service. You have no obligation to us – hidden or otherwise – in checking the boxes and receiving the resulting recommendations. This is part of our commitment to serving the local community. There are only 22 “Yes” or “No” responses required and it should take you less than 8 minutes to complete it. The assessment is designed to test the participant’s awareness of important aging issues. The only personal information that is required is the name of the aging senior, his or her marital status, age and contact information for the person doing the evaluation. That's it. We will take the information you give us and create a report based on your responses. The report identifies important aging issues, some of which you may not even be aware of. The report also contains a short description of each aging issue and possible ways to deal with each one. It is important you give us accurate contact information so that we can arrange to get this report into your hands either through email or surface mail or any other means you choose.

CARE MANAGEMENT
We offer care management which is designed to assist patients and their support systems in coordinating and managing care.
Transitional Care Management focuses on setting up and coordinating a patient’s smooth transition between healthcare settings, to a housing or community setting like when a patient is discharged from a hospital and returns home.
